1. What is the difference between counselling and psychotherapy?

It could be argued that there is no difference. However having originally trained as a counsellor and then as a psychotherapist there was a huge difference in the length and practical and theoretical depth of those trainings.
A practical distinction is that psychotherapy tends to be longer and for more complex difficulties.

2. What are all these qualifications?

There is currently no statutory regulation for counselling or psychotherapy so anyone can call themselves a counsellor or psychotherapist, and many do without meeting the requirements of either UKCP for psychotherapists or BACP for counsellors.
There are many accrediting bodies in the UK and almost a complete alphabet of letters after practitioners' names. My advice is check them out and the practical and theoretical aspects of the training.

3. What happens at the first session?

I do not have a white coat, stethoscope or a couch! I do have off-road parking. I will make you welcome, and offer you a hot or cold drink if you wish.
The first session is for us to see how we get on and hopefully agree a therapeutic goal. That is how you would like to feel or what you would like to do that would mean you no longer needed to come.
It is often encouraging to consider a positive goal before working on issues and problems.
